29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: On or about March 08, 2024 and times prior to, employees were exposed to a struck-by hazard, while operating a Toyota #849 forklift that did not have a working parking brake and was not identified as unsafe by tagging, or locking the controls to render them inoperable, or physically removed from its operation.
