29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. a- 8 Earlington Avenue, Mullica Hill NJ: The company worksite Foreman was exposed to fall hazards when working from a damaged,portable aluminum extension ladder's fly section containing damaged rungs and no feet. Condition observed on or about 03/08/24. b - 8 Earlington Avenue, Mullica Hill NJ: The company worksite Foreman was exposed to fall hazards when working from an aluminum extension ladder that contained damaged rungs, bent side rails, etc.. Condition observed on or about 03/08/24. Dynamic General Services, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(16)), which was contained in OSHA inspection number 1675156, citation number 2, item number 2 and was affirmed as a final order on 01/31/24, with respect to a workplace located at 218-223 Stanton Road in Mount Holly NJ. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
