29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower level by one or more of the following methods: (A) Covers; (B) Guardrail systems; (C) Travel restraint systems; or (D) Personal fall arrest systems: (a) Mezzanine for Fans/Blowers - Employees worked on the mezzanine to service and maintain fan/blower equipment associated with the pneumatic collection system. The grating that comprised the mezzanine platform had an opening, toward the south end, exposing employees to the hazard of falling 13 feet 4 inches to the plant floor below, on or about March 12, 2024. No abatement certification or documentation required.
