29 CFR 1910.119(f)(1)(i)(D):The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in emergency shutdown including the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown was executed in a safe and timely manner. Location: Work Place. Roof equipment On or about February 21, 2024, and at times prior, the employer did not implement effective emergency shut down procedures of the ammonia refrigeration equipment, in that the operating procedures did not provide clear instructions for shutdown of ammonia refrigeration equipment on the roof in the event of an uncontrolled release.
