Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) 10 East Park Drive, Westampton, NJ: Employees were exposed to fall and crush hazards. Two employees were observed utilizing unapproved blue work platforms supported by Powered Industrial Trucks. Employees were at a height of 7 to 8 feet above the ground. In addition, throughout the facility the blue work platforms could be observed loaded with carpets that were being processed. Observed on or about 02/20/2024. b)10 East Park Drive, Westampton, NJ: employees were exposed to fall and crush hazards. An employee was observed utilizing an unapproved blue work platform supported by a Powered Industrial Truck. The tines on the forklift exceeded the length of the work platform changing center of gravity and capacity. Modifications made without approval. Observed on or about 02/20/2024. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.
