29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Jobsite located at 650 Fairmount Ave, Philadelphia, PA 19123 - On or about February 8, 2024, the employer did not provide training in the recognition of hazards for each employee conducting masonry operations from a HEK MSHF mast climber scaffold, thereby exposing employees to fall hazards.
