Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a.) Domtar Paper Company, LLC; 100 West Center Street Johnsonburg, PA 15845- on or about 1/25/2024 and times prior thereto, the employer did not effectively inform contract employees of the energy control/lockout tagout procedures necessary for safely entering and working in the C10 conveyor tunnel areas. Contract employees entered the C10 conveyor tunnel to conduct chip vacuum operations while equipment was operating and were exposed to Hydrogen Sulfide, resulting in loss of consciousness, rescue, and emergency medical treatment. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
