29 CFR 1926.1053(b)(1): Where portable ladders were used to access an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed; the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: On or about January 23rd, 2024, at 8636 Modeno Street, Fort Pierce, Florida, an employee was exposed to a 15-foot fall hazard while accessing a wall with a ladder that was not extend 3 feet above the landing surface nor was secured at its top to a rigid support.
