29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a.) Warehouse - On or about January 4, 2024, the employer failed to ensure that employee(s) who operate forklifts were properly trained, evaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards. An employee used the forklift to push metal product to a shredder. b.) Warehouse - On or about January 11, 2024, the employer failed to ensure that employee(s) who operate forklifts were properly trained, evaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards. An employee used the forklift to move product to the washer. RQL Scrap Metal, Inc. was previously cited for a violation of this occupational safety and health standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 1593852, citation number 1, item number 3, and was affirmed as a final order on November 1, 2022, with respect to a workplace located at 6548 S. Narragansett Avenue, Chicago, IL 60638. RQL Scrap Metal, Inc. was previously cited for a violation of this occupational safety and health standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 1551898, citation number 1, item number 3, and was affirmed as a final order on February 15, 2022, with respect to a workplace located at 6548 S. Narragansett Avenue, Chicago, IL 60638.
