Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) On or about January 2, 2024 and at times prior, the employer did not ensure employees were provided with specific information regarding the types and magnitude of hazardous energy, potential hazards and the means to perform a full and effective removal of hazardous energy present on the 230/460V 3-phase MAC MD40 Airlock, s/n 1100310551-020-1 during maintenance operations. An employee entered the production line 2 area with inadequate training when conducting clean-up activities on or near hazardous equipment. The employee suffered multiple finger amputation to their right-hand index, middle, ring and pinky fingers.
