29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: On or about December 19th, 2023, employees engaged in commercial manufacturing activities, on the production floor of the facility located at 6161 Abbott Drive, Omaha, NE 68110, were exposed to amputation hazards in that employees were using unguarded machinery. Instances include but are not limited to: a. Two employees are initiating the YLM CNC Tube Bending Machine Number TB1, each with an independent foot pedal located within 1 foot of the machine leaving both employees hands free, bending material that did not require two hands to hold during the operation.
