29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. ArcelorMittal Tubular Products USA, LLC - Shelby, Ohio: On or about December 5, 2023, employees were exposed to caught-in and crushing injuries in that the stacker and carriage of the Line 9 Bundler were not locked out prior to setting up the equipment for a changeover. The employer did not ensure that energy control application steps were implemented to control the machine's electrical/mechanical energy sources prior to the servicing task - the employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished. b. ArcelorMittal Tubular Products USA, LLC - Shelby, Ohio: On or about October 1, 2023, employees were exposed to caught-in and crushing injuries in that the stacker and carriage of the Line 9 Bundler were not locked out prior to setting up the equipment for a changeover. The employer did not ensure that energy control application steps were implemented to control the machine's electrical/mechanical energy sources prior to the servicing task - the employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished. ArcelorMittal Tubular Products USA, LLC was previously cited for a violation of this Occupational Safety and Health standard 1910.147(d), which was contained in OSHA inspection number 1556578, citation number 2, item number 1b and was affirmed as a final order on February 17, 2022, with respect to a workplace located at 132 W. Main St., Shelby, Ohio, 44875. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
