29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) On December 12, 2023, At The Rivers Edge Compressor station, 3992 146th Ave NW, Alexander, ND 58831, all authorized employees did not apply locks to the group lockout box exposing them to re-energization and the release of hazardous flammable gas, fire and explosion hazards while doing maintenance.
