29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: On or about December 7, 2024, and times thereafter, an employee was exposed to fall hazards in excess of 25 feet, because the lanyard was attached at the employee's waist instead of at the d-ring located at the employee's back at the shoulder blades, on a commercial construction site in Danville, Illinois.
