29 CFR 1910.133(a)(5): The employer did not ensure that each affected employee used equipment with filter lenses that had a shade number appropriate for the work being performed, for protection from injurious light radiation: (a) On or about January 24, 2024, and at times prior, the employer did not ensure the plasma table operator in the Boiler Shop wore shaded eye protection of at least a shade eight while operating the machine with direct line of sight to the plasma arc. This condition exposed the operator to injurious light radiation.
