29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about November 15, 2023, the employer failed to ensure that each authorized employee received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. Employees were exposed to caught-in and amputation hazards associated with the unexpected energization of the spinning auger blade of the Castec manufactured High-Speed Continuous Sand Mixer, model Celecta-Flo CEL-100 when performing service and maintenance activities such as, but not limited to, cleaning. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
