29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two, and was not installed under the supervision of a qualified person. a) On November 16, 2023 and at times prior thereto, employees connected their lifeline systems to a thin, sheet metal tab in the housing of an HVAC unit.
