29 CFR 1910.1030(f)(1)(ii)(D):The employer did not ensure that all medical evaluations or procedures including the hepatitis B vaccine, vaccination series, post-exposure evaluation, or follow-up, including prophylaxis, were provided according to recommendations of the US. Public Health Service current at the time these evaluations and procedure took place: a) Northbrook Behavioral Health Hospital: On or about 11/7/2024 a Mental Health Associate that is exposed to blood and other potentially infectious materials during routine job tasks, such as being bitten by psychiatric patients during restraints or de-escalation, did not receive the Hepatitis B vaccination according to the U.S. Public Health Service recommendations. The employee received the first two vaccines on 11/15/2021 and 5/11/2022 and never received the third vaccine. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
