29 CFR 1910.95(k)(2):The training program was not repeated annually for each employee included in the hearing conservation program: New Horizons Baking Company located at 211 Woodlawn Avenue in Norwalk, Ohio: On or about November 6, 2023, the employer did not provide employees with training on the effects of noise on hearing, the use of hearing protection, and the purpose of audiometric testing on an annual basis Employees were exposed to hazards associated with noise for instances such as, but not limited to: a) On November 6, 2023, an employee working on muffin line 1 was exposed to noise at 221.3%, measured on 80 dBA threshold, of the permissible daily dose, or an average sound level of 95.8 dBA, as measured over 480 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure of 95.7 dBA. b) On November 6, 2023, an employee working at the bun packer location was exposed to noise at 326.3%, measured on 80 dBA threshold, of the permissible daily dose, or an average sound level of 99.1 dBA, as measured over 440 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure of 98.5 dBA accounting for the period of 40 minutes unmonitored. c) On November 6, 2023, an employee working at the bulk muffin location was exposed to noise at 97.7%, measured on 80 dBA threshold, of the permissible daily dose, or an average sound level of 89.6 dBA, as measured over 497 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure of 89.8 dBA. d) On November 6, 2023, an employee working on the bun side was exposed to noise at 592.3%, measured on 80 dBA threshold, of the permissible daily dose, or an average sound level of 103.0 dBA, as measured over 465 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure of 102.8 dBA accounting for the period of 15 minutes unmonitored.
