29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information. On or about October 24, 2023 in the Plant 1 Refrigeration Engine Room at Lone Star Bakery located in China Grove, Texas; employees were exposed to fire, explosion and toxic gas release hazards when the employer did not establish the Car Seal Procedure to ensure the block valves on relief lines were maintained properly in the desired operating position.
