29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. On or about October 18, 2023, at a jobsite located at 1661 Rush Rd. Wickliffe OH 44092; the employer did not ensure inspections were conducted by a competent person. The depth of the trench was determined to be 6ft 4in. and while installing a sewer line, an employee was exposed to possible cave in hazards due to the lack of cave-in protection.
