29 CFR 1910.244(a)(2)(iii): Loads were not cribbed, blocked or otherwise secured immediately after being raised with a jack: a. On or about September 27, 2023, at Locomotive Railserve 8216 Mod GP-9 at the USD terminal Track A spur, 719 Richard James Avenue, Stroud, OK. 74079, locomotive traction control motor #3 was not secured from falling immediately after it was raised with a bottle jack. An employee worked under the locomotive traction control motor #3 and was exposed to crushed-by hazards. b.On or about September 27, 2023, at Locomotive Railserve 8216 Mod GP-9 at the USD terminal Track A spur, 719 Richard James Avenue, Stroud, OK. 74079, locomotive traction control motor #4 was not secured from falling immediately after it was raised with a bottle jack.
