29 CFR 1910.180(b)(3): The employer did not ensure only designated personnel were permitted to operate a crane covered by this section: (a) (a) Flat deck barge #4, Sumitomo SC 800 crawler crane: On or about 9/19/2023, and at times prior, the employer exposed employees to roll-over hazards, crushing hazards, and drowning, in that the crane was operated by an uncertified employee who was still in training without a supervisor present.
