29 CFR 1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a. BlueScope Recycling and Materials, LLC - Mansfield, Ohio: On or about September 7, 2023, the employer did not ensure that employees who maintained the Transfer Line 4 waste conveyor, located outside the Insulated Copper Wire (ICW) building, were trained in the skills required for the safe application, usage, and removal of energy controls. An employee who performed cleaning activities sustained an arm amputation injury after being caught in the rotating tail pulley; the employee was not trained to isolate and/or render energy sources safe. b. BlueScope Recycling and Materials, LLC - Mansfield, Ohio: On or about September 7, 2023, the employer did not ensure that employees who maintained the Transfer Line 4 waste conveyor, located outside the Insulated Copper Wire (ICW) building, were trained in the skills required for the safe application, usage, and removal of energy controls. Employees who performed cleaning activities were not trained to isolate and/or render energy sources safe. c. BlueScope Recycling and Materials, LLC - Mansfield, Ohio: On or about September 1, 2023, the employer did not ensure that employees who maintained the Transfer Line 4 waste conveyor, located outside the Insulated Copper Wire (ICW) building, were trained in the skills required for the safe application, usage, and removal of energy controls. Employees who performed cleaning activities were not trained to isolate and/or render energy sources safe. d. BlueScope Recycling and Materials, LLC - Mansfield, Ohio: On or about September 7, 2023, the employer did not ensure that employees who maintained the Transfer Line 4 waste conveyor, located outside the Insulated Copper Wire (ICW) building, were trained in the skills required for the safe application, usage, and removal of energy controls. Employees who applied grease to the equipment's tail pulley were not trained to isolate and/or render energy sources safe. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
