29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not address human factors: On or about December 8, 2023, the PHA revalidation for the Soy East Extraction Process did not adequately address human factors. a. Addressing instances where exterior doors were propped open which would prevent the ventilation system from capturing fugitive Hexane from leaving the process into unprotected areas outside of the process; b. Addressing instances where standard operating procedures were not being followed in sequential order to prevent Hexane from entering upstream connected equipment in the Prep Building.
