29 CFR 1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. Location: Jobsite 780 West Main Street, Avon, MA On August 23, 2023, employees were exposed to falls from heights up to 13 feet to the ground while accessing an upper landing surface where a materials hoist was used to access a commercial roof and the employees were not trained in recognizing and minimizing hazards related to working from ladders.
