29 CFR 1910.23(c)(9): The employer did not ensure that portable ladders that are used on slippery surfaces are secured and stabilized. a) Extension Ladder against Phosphoric Acid Holding Tank in Production Area; On or about August 22, 2023, and at times prior, the employer exposed employees to fall hazards, in that employees are allowed to use an extension ladder approximately 13-foot 1 inch in height to check the levels of the holding tanks on a slippery floor created by chemical residues.
