29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about August 21, 2023, at the establishment located at 3575 N. Main Street, Oshkosh, WI, a Machine Operator was exposed to machine hazards associated with ingoing nip points while working inside the Torin W-24S Coil Former Front Door area. The Torin W-24S was not locked out and alternative measures as effective as machine guarding had not been implemented during setup while working inside the Torin W-24S Coil Former Front Door area. b) On or about November 22, 2023, at the establishment located at 3575 N. Main Street, Oshkosh, WI, a Machine Operator was exposed to machine hazards associated with ingoing nip points while working inside the Torin W-25 Coil Former feed gears enclosure. c) On or about August 21, 2023, at the establishment located at 3575 N. Main Street, Oshkosh, WI, a Trimmer Operator did not unplug and control the power cord of the Trimmer #1 stand grinder while replacing the cutoff wheel. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.
