29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about August 15, 2023, employees changing dies or maintaining equipment including but not limited to the RATCHET machines (20D155, 2000, 20MFX1, 20MFX2, and 20MFX3), HARDINGE machines (22LBF1, 22LBF2, 22LBF3, 22LBF4, 22LBF5, 22LBF6, 22LBF7, 22NC701, 2200, and 22HARD), OMNIS lathes (13C1179, 13LTF1, 13LTF2, 13LHF5, 13LHF6, 12NC110, 12NC900, and 13NC906), and WADCO machines (25P3, 25p997, and 25R1299) in Mazaks (2000), CNC Lathes (2200), Lathe (1300), Omni (1300), Finishing (1700), and Assembly (2500) were exposed to the release of potential electrical (240volts-480volts), and pneumatic energy; the employer did not ensure each machine had clear and specific procedures to outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy.
