29 CFR 1926.502(d)(8):Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: a) Construction site, Sterigere St. Bridge, Norristown PA - On or about August 2, 2023, a horizontal lifeline provided for employee use as fall protection was not installed and used under supervision of a qualified person and did not meet specifications contained in the engineered design drawings, exposing employees to serious injury in the event of failure.
