29 CFR 1910.140(c)(13)(ii): Anchorages were not designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two: a) On or about July 26, 2023, and times prior to, located at the R and S Loading Rack, employees working from an elevated Boom Trac platform with open hatches were exposed to a fall hazard when using a metal guardrail as a fall protection anchorage point that had not been evaluated by a qualified person.
