29 CFR 1910.28(b)(6)(i): The employer did not ensure that each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard. a) United Premium Foods, LLC./1 Amboy Avenue/Woodbridge, NJ 070955 - Sausage Room: Employee(s) such as machine operator(s) used a two-tier stairway atop a work platform at the front of the AMFEC Vacuum Mixer Model #510, Serial #2012014 for greater working height and closer access to the open top of the mixer. Their elevated position atop the stairway defeated the fall protection afforded by the work platform guard-rail system and exposing them to hazard of falling into rotating machine parts. Observed on or about on 07/28/2023.
