29 CFR 1910.303(h)(2)(iii)(A): The installations shall be made with metal-enclosed equipment or shall be enclosed in a vault or in an area to which access is controlled by a lock; a) On or about July 25, 2023, technicians inspecting the mine were exposed to electrical shock and arc flash hazards due to the vaults not being controlled by a lock; the employer did not ensure the vault were locked to other than qualified persons exposing technicians to electrical shock and arc flash hazards.
