29 CFR 1910.147(c)(5)(i):Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: On or about July 22, 2023 and times prior to, at a commercial jobsite located at 7115 Clinton Drive, Houston, TX 77020, on blending line #10, ribbon auger blender, the employer failed to provide locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware to isolate, secure, or block the ribbon blender shaft from all energy sources.
