29 CFR 1926.959(d)(2):A designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment and provide timely warnings such that the minimum approach distance was being maintained before the minimum approach distance required by paragraph (d)(1) of this section was reached: (a) At 5351 SR 69, Cullman, AL: On or about 7/12/2023, the employer exposed employees to electrical hazards in that, a designated observer did not warn employees performing tasks in close proximity to three phase, 14400 V energized primary, such as but not limited to, connecting underground secondary to a vertical pole mounted switch.
