29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s): On or about October 12, 2023, the employer did not ensure that the Yoder 12-Stand Roll Former (Serial 103162) on the PC3 line was guarded to protect against crushing and amputation injuries. The ingoing nip points created by the roll forming dies did not have any guarding to prevent employees from accessing the zone of danger. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
