29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not de-energized and grounded, nor were other protective measures provided before work was started: At the residential address located at 201 East Tripp Street in Mansfield MO 65708 on or about July 7, 2023: Employees performing telecommunications work were exposed to burns, shocks, and electrocutions when working near energized overhead power lines. The employee was working approximately 15 to 20 feet above the ground, inside the bucket of a lift, below uncovered energized power lines. The employee was manipulating a loose copper wire. Contact or arcing between the energized line and copper wire resulted in burns and electric shock, fatality injuring the employee.
