29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. a) 1040 Hwy 35, Middletown, NJ: The employer did not provide training covering electrical hazards from equipment approaching or contacting energized power lines. The employee encroached on power lines making contact. Violation occurred on or about 07/11/23 b) 1040 Hwy 35, Middletown, NJ: The employer did not provide training covering fall hazards from operating an articulating boom lift. The Employee did not wear fall protection while at a height of approximately twenty one feet. Violation occurred on or about 07/11/23 NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
