29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for respirator use: On or about July 7, 2023, and at times prior thereto, employees were cleaning crushed glass and other debris while wearing a tight-fitting half-mask respirator without the implementation of a written respiratory protection program, medical evaluation, fit testing, or annual training, exposing employees to the hazard of an undue physiological burden.
