29 CFR 1910.177(f)(10):The employer did not ensure that employees stayed out of the trajectory whenever multi-piece rim wheels are handled: On or about 06/21/2023, in the maintenance shop located at 40096 Runway Blvd, Pratt, KS 67124, employee(s) were exposed to struck-by hazards in that it was not ensured that employees worked outside of the multi-piece rim wheel trajectory. The multi-piece piece wheel exploded following the removal of three of nine-wheel studs. An employee suffered fatal injuries as a result of the incident.
