29 CFR 1910.242(a):Hand and portable powered tools or equipment were not kept in safe condition: a) HK Cooperative DBA J.H. Routh Packaging Co. on or about June 29, 2023, at the address of 4413 Bogart Rd, Sandusky, OH: The employer failed to ensure the employer furnished Hercules trigger grip angle grinder (model #HE65) was inspected for safe condition prior to employee use. Employees tasked with using the tool without appropriate factory guarding was exposed to the rotating wheel during metal grinding/cutting operations.
