29 CFR 1910.119(h)(2)(i): The employer, when selecting a contractor, did not obtain and evaluate information regarding the contract employer's safety performance and program: At the establishment located at 3019 Scanlan Avenue, Salina, Kansas, the employer failed to obtain the safety and health programs of the ammonia contactor that delivers and participates in the unloading of highly hazardous chemicals into the ammonia refrigeration system. The driver of the ammonia tanker truck used their company's hoses and equipment to transfer the highly hazardous chemical. Employees engaged in operating, inspecting, testing, and maintaining activities in the ammonia refrigeration system at the facility were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury.
