29 CFR 1910.212(a)(5):Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: a) Toledo Molding & Die, LLC. On or about June 8, 2023, at the address of 515 Gypsy Lane Rd. Bowling Green, OH: The employer did not ensure industrial pedestal fans located in production facility were adequately guarded. Employees adjusting/moving fans with blade guard openings up to 3/4 inch were exposed to amputation hazards.
