29 CFR 1910.119(j)(3): The employer did not train each employee involved in maintaining the on-going integrity of process equipment in an overview of that process and its hazards and in the procedures applicable to the employee's job tasks to assure that the employee can perform the job tasks in a safe manner. On or about May 23, 2023, at the Wynnewood Refinery in Wynnewood Oklahoma, employees were exposed to fire and chemical release hazards when the employees were not trained to perform maintenance work on the automated valve of the chemical process equipment.
