29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Roadway in Front of 100 Olmstead Drive, Middletown, Pennsylvania - Employees worked in the excavation, which had a depth of 7 feet 5 inches, dug in Type B soil, to install new storm sewer piping and structures. This excavation did not have protective systems for Type B soil on the north, south, and east walls, exposing the employees to a cave-in hazard, on or about May 22, 2023 Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
