29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment shall be consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience. On or about May 15, 2023 at Marathon Galveston Bay Refinery in Texas City, TX the employer did not ensure that inspections and testing performed on process equipment follow recognized and generally accepted good engineering practices, and more frequently if determined to be necessary by prior operating experience when vibration analysis was not conducted in accordance with, but not limited to, Galveston Bay Refinery's inspection, testing, frequency of preventative maintenance for pumps included in the Mechanical Integrity program, including 303-J as detailed in Appendix D, Matrix 2 of RSP-1308 PSM/RMP Mechanical Integrity.
