29 CFR 1910.243(c)(5)(i): The spindle speed of the machine was not checked before mounting of the wheel to ensure that it did not exceed the maximum operating speed marked on the wheel: Location: Sprinter Shop On or about April 18, 2023, employees were exposed to struck by hazards from flying particles when a 4" reinforced cut-off wheel rated for 19,100 rotations per minute (rpm) was mounted to a Mac Tools AG325S ?" die grinder rated for 30,000 rotations per minute (rpm) and was used to cut metal.
