29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Precision Strip, Inc. - Perrysburg, Ohio: On or about April 14, 2023, Employees who were tasked with clearing jams inside line 905, where hazardous energy sources were present, such as but not limited to those created by the energized conveyors, pneumatic flipper arms, hydraulic conveyor lifts, and hydraulic stacker cars were exposed to caught in hazards. The employer did not ensure energized components of the aforementioned line were effectively controlled or isolated, prior to bodily entry into the line's gated area.
