29 CFR 1910.269(n)(6)(i): Employees were allowed to attach the testing end of grounding cables without attaching the ground-end connection first: On or about March 22, 2023, at a remote location in the backyard of a personal residence, while connecting the cables of fault locator to the cables in a transformer, the employee at the transformer contacted the cables in the transformer before the employee on the opposite end attached the ground cable, exposing employees and contract employees to the hazard of coming in contact with energized/ungrounded parts.
