29 CFR 1910.178(o)(6): Extreme care was not used when tilting the load forward or backward. Tilting forward with load engaging means elevated was not prohibited except to pick up a load. An elevated load was tilted forward when the load was not in a deposit position over a rack or stack. a) yard outside machine shop: On or about February 23, 2023, a large valve was rigged with the loops of the lift strap attached to the fork tine when the operator attempted to place the load on a sawhorse by slightly tilting the forks, the straps slipped on the forks resulting in a struck by hazard and the part falling onto an employees foot.
